  • Use color Doppler to distinguish vessels from bowel (vessels show flow, bowel doesn't)
  • Confirm psoas by asking patient to flex hip - muscle contracts and changes appearance
  • Trace iliac vessels from external iliac to common iliac to confirm identity
  • Look for appendix 'sliding sign' - moves independently from surrounding structures with gentle compression
  • Document all four landmarks in your report even if appendix not visualized
